DATA PRIVACY CONSENT FORM

This consent acknowledges Urdaneta City University’s (UCU) right collect, process, record, organize, update, or modify, retrieve, share, consult, use, block, and erase my personal and sensitive data (in any form, including but not limited to text, numeric, photographic, digital and other forms) as part of my personal information in the pursuit of its legitimate interest and academic functions.

In order to get in touch with me regarding my pre-admission, enrollment, and other lawful school-related activities, I give UCU permission to gather and utilize my personal information, including but not limited to directory information.

In accordance with the provisions of Republic Act No. 10173 of the Philippines, Data Privacy Act of 2012, and its corresponding Implementing Rules and Regulations. I also affirm my right information, access, update, rectify, obtain a copy, erasure, or blocking of my personal data, object to processing, and receive compensation in the event of damages. I also affirm my right to file a complaint.

I have read and understood the contents of this form. I further agree that the data privacy consent is binding from the date signed and during my stay in Urdaneta City University
